Purpose

Spravato® (esketamine) may cause sedation, dizziness, impaired attention, judgment, and coordination. For patient safety, patients receiving Spravato must have appropriate transportation arranged after each treatment session.

Transportation Requirements

• Patients must arrange for a responsible adult to drive them home after each Spravato treatment.
• Patients may not drive themselves home after receiving Spravato.
• Patients should not drive, operate machinery, or engage in activities requiring full alertness until the following day after a restful night's sleep, or as otherwise directed by their treating clinician.
• The driver should be able to safely transport the patient directly home following treatment.
• Rideshare or taxi transportation without a responsible adult accompanying the patient may not be used unless specifically approved by the treating clinician and permitted by clinic policy.
• The clinic may require the patient to remain onsite until they are clinically appropriate for discharge.
• If the patient arrives without appropriate transportation or the arranged driver is unavailable, the treatment may need to be postponed or rescheduled.

Patient Responsibilities

I understand that I am responsible for arranging safe transportation before each Spravato treatment. I agree to notify the clinic as soon as possible if my transportation arrangements change or if I am unable to arrange an appropriate ride home.

I understand that the clinic may verify transportation arrangements before or after treatment and may decline to administer or discharge me from treatment if appropriate transportation cannot be confirmed.

Emergency/Unexpected Circumstances

If my designated driver becomes unavailable after treatment, I will notify clinic staff immediately. I understand that clinic staff cannot provide transportation home unless specifically authorized by the clinic.

Acknowledgment

By signing below, I acknowledge that:

• I have been informed of the clinic's transportation requirements for Spravato treatment.
• I understand that I cannot drive myself home after treatment.
• I understand that Spravato can temporarily impair alertness, judgment, coordination, and other abilities needed for safe driving.
• I agree to arrange appropriate transportation for each treatment session.
• I understand that failure to comply with this policy may result in treatment being postponed or rescheduled.
• I have had an opportunity to ask questions and have received answers that I understand.
