We understand that patients may occasionally need supporting documentation such as letters, FMLA paperwork, or medical records. Please review our policy below regarding fees, processing times, and delivery procedures.
How to Request Completion of Your FMLA or Disability Forms

Form Fees
- General Letters: $50
- FMLA/Disability: $100
- Medical Records(Preparation Fee): $75
Please note: All fees must be paid in advance before processing begins.
Processing Time
- All letters, forms, and records requests require up to 14 business days to process.
- We do not offer same-day or expedited services.
- FMLA or Disability Paperwork is NOT completed same day.
- Requests are reviewed and completed solely at the provider's discretion.
- FMLA/Disability paperwork is only available for established patients — we do not complete these forms for new patients or those not actively receiving care from our practice.
Delivery and Pick-Up Policy
- In accordance with HIPAA guidelines, we do not email letters, forms, or medical records.
- All completed documentation must be picked up in person from our office or faxed.You will be contacted when your documents are ready for pickup.
Services We Do Not Provide
- Emotional Support Animal (ESA) Letters
- Forensic Evaluations or Reports for:
- Criminal cases
- Civil litigation
- Family law or custody cases
- Immigration cases
- Fitness-for-Duty Evaluations for employment or legal purposes
- Second Opinions for Legal Disputes or Disability Appeals
- Medication refills for patients not actively engaged in treatment
- Crisis or Emergency Services – If you are experiencing an emergency, please call 911 or go to your nearest emergency room.
Our practice is focused on providing ongoing psychiatric care, medication management, and support within the therapeutic scope of outpatient mental health treatment.
